Scleroderma is a group of disorders in which inflammation and excessive collagen lead to fibrosis, making affected skin or other tissues unusually hard, thick or tight. The fibrotic tissue is less flexible than normal tissue, so visible skin change can be accompanied by stiffness, reduced movement, vascular problems or other symptoms depending on the disease type.

The first clinical distinction is whether the disease is localized scleroderma—most often morphea, which affects skin and sometimes deeper nearby tissues—or systemic sclerosis, an autoimmune disease that can also involve blood vessels and internal organs. Treatment therefore depends on the manifestation being treated, and systemic sclerosis requires ongoing organ monitoring even when the skin appears stable.

Scleroderma causes hardened or tight tissue because abnormal immune and vascular activity can activate fibroblasts, leading to excessive collagen deposition and fibrosis.

How Does Excess Collagen Cause Fibrosis?

Excessive collagen accumulation makes affected tissue thicker, firmer and less flexible as normal tissue architecture is altered by fibrotic repair.

In systemic sclerosis, the process is commonly described as interacting immune dysregulation, blood-vessel injury and fibroblast activation. Fibroblasts then produce excess collagen and other extracellular-matrix proteins, which accumulate in skin and, in systemic disease, may also affect internal organs.

Is Scleroderma an Autoimmune Disease?

Systemic sclerosis is an autoimmune connective-tissue disease involving immune abnormalities, vascular dysfunction and fibrosis, although the event that first triggers the disease remains uncertain.

The broader word “scleroderma” also includes localized disease. That distinction matters because localized forms can extend into nearby fat, fascia, muscle or bone without producing the characteristic internal-organ vasculopathy and fibrosis of systemic sclerosis.

What Does Scleroderma Look and Feel Like on the Skin?

Scleroderma can cause puffy fingers, progressively tight or shiny skin, reduced flexibility, pigment change, dryness, itching and difficulty bending affected joints.

Fibrotic skin may feel firm or bound down and lose normal folds. Hair and sweating can decrease within involved areas, and tightening around the fingers, hands, mouth or joints can become functionally important rather than being only a cosmetic change.

What Are Sclerodactyly, Telangiectasia and Calcinosis?

Sclerodactyly describes tightened fibrotic skin of the fingers, telangiectasia refers to visible small dilated blood vessels, and calcinosis refers to calcium deposits in or beneath the skin.

These findings do not all appear in every person, but they help describe the pattern of systemic sclerosis when interpreted together with Raynaud phenomenon, digital ulcers, skin distribution and organ findings.

What Types of Scleroderma Affect the Skin and Internal Organs?

The most important distinction is between localized scleroderma, which affects skin and sometimes deeper nearby tissues, and systemic sclerosis, which also carries vascular and internal-organ risk.

What Is Localized Scleroderma or Morphea?

Localized scleroderma causes fibrotic patches or bands in the skin and can extend into nearby fat, fascia, muscle or bone, but it does not produce the characteristic internal-organ fibrosis or vascular injury of systemic sclerosis. Because depth matters, “localized” should not be read as “always superficial.” Linear or deep morphea can affect growth, joints or movement even though the disease remains different from systemic sclerosis.

What Is Systemic Sclerosis?

Systemic sclerosis combines autoimmunity, blood-vessel abnormalities and fibrosis and may affect both the skin and internal organs.

Possible complications include interstitial lung disease, pulmonary arterial hypertension, esophageal and other gastrointestinal dysfunction, cardiac disease, digital ischemia and kidney involvement. Not every person develops every manifestation, which is why baseline assessment and follow-up are individualized.

What Is the Difference Between Limited and Diffuse Systemic Sclerosis?

Limited cutaneous systemic sclerosis generally has skin involvement distal to the elbows and knees, often including the hands and face, while diffuse cutaneous disease extends more proximally onto the upper arms, thighs or trunk.

“Limited” describes the skin distribution; it does not mean harmless disease. People with limited cutaneous systemic sclerosis can still develop important pulmonary vascular, lung, gastrointestinal and other internal-organ complications, while diffuse disease tends to carry a different timing and pattern of organ risk.

How Can Raynaud Phenomenon Be an Early Systemic-Sclerosis Sign?

Raynaud phenomenon can precede other systemic-sclerosis signs and causes recurrent cold- or stress-triggered finger color changes from temporary narrowing of small blood vessels.

What Causes Scleroderma and How Is It Diagnosed?

Scleroderma does not have one proven universal cause, and diagnosis depends on combining the clinical pattern with targeted laboratory, vascular and organ assessment rather than relying on one test.

Is There One Known Cause of Scleroderma?

No; scleroderma appears to arise from interacting immune, vascular, fibrotic, genetic and environmental influences rather than from one infection, food or exposure.

Research supports a model in which susceptible tissue develops abnormal immune activation and vascular injury, which can stimulate prolonged fibroblast activity and excessive extracellular-matrix production. The exact initiating sequence varies and remains incompletely understood.

How Does a Doctor Diagnose Scleroderma?

Scleroderma diagnosis combines symptom history, skin examination, vascular findings, physical examination and targeted testing, followed by organ assessment when systemic sclerosis is suspected.

The clinician asks whether hard or tight skin fits localized disease or the distribution of systemic sclerosis, whether Raynaud or fingertip vascular injury is present, and whether symptoms point toward lung, gastrointestinal, heart or kidney involvement. No single laboratory or biopsy result replaces that clinical synthesis.

Which Blood Tests and Vascular Tests Can Support Diagnosis?

ANA and disease-associated autoantibodies can support systemic-sclerosis assessment, while nailfold capillaroscopy can identify characteristic small-vessel abnormalities in selected people with Raynaud symptoms.

A positive ANA is not enough to diagnose systemic sclerosis because antinuclear antibodies can occur in other autoimmune diseases and in some people without systemic sclerosis. Autoantibody pattern, symptoms, skin findings and vascular changes must be interpreted together.

What Conditions Can Resemble Scleroderma?

Other autoimmune and fibrosing disorders can produce overlapping skin, vascular or musculoskeletal findings, so an atypical hardening pattern deserves a controlled differential rather than automatic labeling as scleroderma.

Lupus erythematosus can overlap with systemic autoimmune symptoms and vascular complaints, but it has a different disease pattern and should remain a separate diagnostic pathway.

Dermatomyositis becomes a more relevant differential when characteristic skin findings occur together with prominent muscle weakness or other myositis features.

How Are Internal Organs Checked in Systemic Sclerosis?

Systemic-sclerosis assessment is directed toward organs known to be at risk, particularly the lungs, pulmonary circulation and heart, kidneys, and gastrointestinal tract.

How Is Scleroderma Treated?

Scleroderma treatment is manifestation-specific, meaning skin fibrosis, vascular disease, digestive symptoms, lung disease and movement restriction may each require different therapies.

Can Scleroderma Be Completely Cured?

There is currently no single treatment that cures every form of scleroderma, so therapy focuses on controlling active disease, relieving symptoms, preserving function and preventing or limiting organ damage.

Some inflammatory or early fibrotic changes can improve, but established fibrosis is not guaranteed to reverse completely. Treatment expectations should therefore focus on measurable disease control and function rather than promising normal tissue restoration.

How Is Skin Thickening or Active Skin Disease Treated?

Active skin disease is treated according to the scleroderma branch: selected localized morphea may respond to topical therapy, phototherapy or systemic immune-directed treatment, while systemic-sclerosis skin fibrosis may require rheumatology-directed immunomodulatory therapy.

Current systemic-sclerosis recommendations include options such as methotrexate, mycophenolate mofetil or rituximab for selected skin fibrosis, with treatment chosen around disease activity, organ involvement, comorbidities and specialist judgment. How Are Raynaud Phenomenon and Finger Ulcers Treated?

Treatment of scleroderma-related vascular disease aims to reduce vasospasm, improve circulation, protect the fingers and promote healing of digital ulcers.

How Can People With Scleroderma Protect Their Skin, Monitor the Disease, and Know When to Seek Urgent Care?

Daily skin protection, cold avoidance, movement exercises and long-term organ monitoring help reduce complications, while new breathing, kidney or severe circulation symptoms require prompt assessment.

How Can Moisturizing and Gentle Skin Care Help?

Harsh scrubbing and strongly drying products can worsen discomfort in already tight or fragile skin. Skin care supports comfort and barrier protection, but it does not replace disease-directed treatment for active fibrosis.
